How to Have Genetic Testing Performed?

A sample of biological material - blood or saliva - is necessary to perform a genetic test. Almost all of the tests available in NZOZ Genomed are carried out using saliva. Only some of the tests require collecting venous blood into a tube.
Following a customer’s request, Genomed sends a special kit, adapted to the ordered test and a patient’s needs, which includes a tube for collecting saliva with a lid containing a stabilizing liquid. The price of the kit is 50 PLN, and it does not include the cost of shipping: kit to customer, sample to NZOZ Genomed.
Before ordering the selected genetic test, please familiarize yourself with the accompanying information concerning the kind of biological material required.
ATTENTION! Karyotype testing (KAR-1 and NP-5) is carried out only using venous blood collected into a tube containing heparin.
To order the kit call us (+48 22 498 2 498,+48 22 644 60 19), contact us via e-mail (diagnostyka@genomed.pl) or, preferably, fill in and send us the on-line form .
The kit will be sent after the test has been paid for.

Please provide us with complete patient identification data, including full name, date of birth, sex, ethnic origin, nationality, as well as relevant clinical information. Please do it as thoroughly and carefully as possible to avoid making registration mistakes. Remember: The more precise the clinical data provided, the more comprehensive the interpretation of the obtained testing results!
The collected biological material along with the Request for Genetic Testing and Declaration of Informed Consent or the Declaration of Informed Consent and the Order Form for Genetic Testing (when the test is not paid for by the patient) should be sent to the following address:

The collected biological material along with the filled-in forms should be sent by courier delivery service or, possibly, by the priority mail of the Polish mail service (Poczta Polska). Shipment costs are covered by the customer. Genomed will not be held responsible for the loss of the shipment sent to the company.

The genetic testing result will be sent by mail to the address given in the form. There is a possibility of sending the result by courier delivery service (DHL, 250 PLN) – the cost is then covered by the ordering customer.
Collecting the results in person: at NZOZ Genomed, ul. Ponczowa 12, Warsaw, Poland.
Individual customers (not referred to our institution by a company cooperating with us) are kindly requested to make the payment before the testing is performed.
